Overview

IV push medication preparation remains a persistent source of risk in anesthesia practice. Unlabeled syringes, variable dose preparation practices, and workflow-related vulnerabilities continue to contribute to medication errors in perioperative settings, despite sustained attention to the problem.

In this breakfast symposium at ANESTHESIOLOGY® 2026, faculty will draw on findings from the 2026 white paper IV Push Medication Practices: Identifying Persistent Gaps and Advancing Safety, analyses of medication error reports, and real-world perioperative safety examples to describe where these vulnerabilities persist. Speakers will compare manufacturer-prepared ready-to-administer (RTA) products, pharmacy-prepared RTA products, and traditional vial-and-syringe preparation, and discuss practical strategies to advance the hierarchy of injectable medication safety in anesthesia practice.

Learning objectives

  • Describe persistent gaps in IV push medication practices and associated risk-mitigation strategies
  • Compare manufacturer-prepared ready-to-administer products, pharmacy-prepared ready-to-administer products, and traditional practitioner-prepared vial-and-syringe processes within the hierarchy of injectable medication preparation safety.
  • Examine the impact of injectable medication dosage forms, concentrations and preparation methods of anesthesia workflow and patient care
  • Analyze evidence demonstrating the impact of ready-to-administer (RTA) products on patient safety and clinical outcomes.
