Overview
The Top 10 2025 edition introduces several new topics, focusing on emerging risks that have the potential to significantly impact patient safety. To identify where vulnerabilities exist, healthcare leaders must assess every component of the system—people, organizations, tasks and processes, tools and technology, and the physical environment. Each concern in this year's report reflects a breakdown in at least one of these areas, with many issues overlapping and originating from multiple sources.

Total systems safety focuses on creating greater efficiency and resilience in clinical and safety operations. It incorporates principles of clinically informed human factors engineering, just culture, health equity, and advanced safety science to redesign individual components of systems to be more transparent and aligned—leading to safer and more effective care.
