
Using examples of events that have been reported to the ISMP National Medication Errors Reporting Program (ISMP MERP), data from cost-effectiveness research studies, and first-hand experience shifting an organization toward use of ready to administer (RTA) products, the faculty describes the key vulnerabilities when IV medications require manipulation at the bedside that have led to errors and patient harm, show listeners how they can review their own data to compare the cost and safety of manufacturer-prepared RTA products compared to traditional vial-and-syringe products, and illustrate the steps needed to successfully optimize use of RTA products in their organizations.
