Safe IV Push Practices Amid Drug Shortages

A Midday Symposium conducted at the 2025 ASHP Midyear Clinical Meeting and Exhibition

Speakers will review potential adverse drug events (ADEs) associated with IV push medication preparation and administration, particularly in the context of IV fluid shortages. This will include survey results of current IV push practices, safety concerns, and risks associated with administration. Speakers will discuss updated safe practice guidelines and system-level strategies to prevent medication errors and support safe IV push administration.

Speakers

Rita Jew, PharmD, MBA, BCPPS, FASHP
President, ISMP

Rita K. Jew, PharmD, MBA, BCPPS, FASHP, is President of the Institute for Safe Medication Practices (ISMP) and brings over 30 years of leadership in pharmaceutical care. Her career spans executive roles at leading institutions such as UCSF Health and the Children’s Hospitals of Orange County and Philadelphia, as well as consulting through her firm RKJ Health Partners. Dr. Jew has contributed extensively to the field through publications, presentations, and co-authorship of a book on extemporaneous formulations. A recognized leader in professional pharmacy organizations, she has received multiple honors, including the Jonathan Roberts Award and ASHP’s Distinguished Service Award. She holds a PharmD from UCSF, an MBA from Wharton, is a Board-Certified Pediatric Pharmacy Specialist, and completed a clinical pharmacy residency at Thomas Jefferson University Hospital.

Jana O'Hara, MSN, RN, CPHQ, CPPS
Director, Consulting & Education, ISMP

Jana O’Hara, MSN, RN, QPHQ, CPPS is the Director of Consulting and Education at ISMP. Jana has worked in a variety of clinical leadership, quality & safety roles.  Most recently, she served as the Director of Marketplace Operations for a healthcare staffing company, where she led both clinical and non-clinical teams and developed corporate policies and procedures to promote safe clinical practices, evaluation of staff, and ongoing regulatory compliance. Prior to this she served as Director of Patient Safety for University Health in San Antonio, TX, overseeing patient safety across the entire healthcare system including inpatient, ambulatory, ASC, dialysis and correctional facilities. It was during this time that Jana’s passion for medication safety grew, as well as her dedication to work towards creating a healthcare environment with zero harm.  She has a BS in Nursing from Biola University and an MS in Nursing Systems Management with a Graduate certificate in Nursing Education from University of Texas at El Paso.
